Our team of hospice doctors, nurses, home health aides, volunteers, and therapists is dedicated to providing the highest degree of in-home hospice care for our patients facing terminal illnesses. With palliative care nursing, dementia care, and bereavement services, we do our utmost to enhance the physical, mental, spiritual, and emotional well-being of our patients during their time with us.

Hospice Services

We believe that patients should be apprised of their clinical prognosis and be involved in the decision-making process that determines how their end-of-life care is provided.

ROUTINE HOME CARE

Hospice Group of Southern California, Inc. provides residential care in patients’ homes and has established partnerships with long-term care facilities and nursing homes, assisted living facilities and residential care facilities for the elderly.

CONTINUOUS CARE

If a patient develops the need for continuous care, we can provide shifts of hospice staff, including palliative care doctors and nurses, at the patient’s home for a brief period so the patient can avoid being transferred to a hospital.

DEDICATED INPATIENT UNITS

Hospice Group of Southern California, Inc. has special arrangements with local hospitals and other facilities if patients require additional support that can’t be provided at home.

RESPITE CARE

Limited to five consecutive days, respite care provides a period of rest for the patient’s primary caregiver. Home care patients will be admitted to an inpatient facility without having to meet the normal pain and symptom management criteria.

TELECARE

Hospice Group of Southern California, Inc. Nurse is just a phone call away 24 hours a day, 7 days a week. Our hospice care providers will conduct home visits whenever necessary to respond to crises related to illness.

CERTIFIED HOSPICE AIDES

Our hospice and palliative doctors, nurses, and support staff assist with activities of daily living and help with personal care, such as bathing and oral hygiene. They also can help with light cleaning, laundry, and occasional shopping.
